CMS CMMI ACCESS Program
The ACCESS Resource Center is the central hub of all information related to the CMS CMMI ACCESS innovation program. Advancing Chronic Care with Effective,
Scalable Solutions (ACCESS) Model, a new Center for Medicare and Medicaid Innovation
(Innovation Center) model from the Centers for Medicare & Medicaid Services (CMS). The
ACCESS Model will test whether an alternative payment methodology—Outcome-Aligned
Payments (OAPs)—for technology-enabled chronic care reduces expenditures while
preserving or enhancing quality of care for Medicare beneficiaries.
The ACCESS Model is designed to expand choice for patients and clinicians by offering a
new payment option that removes payment barriers to technology-enabled care
